The Standing Rock Sioux Tribe will hold talking circles and lead a community assessment to guide the development of a network of home and community-based service providers who qualify for Medicare and state reimbursement programs. This network will help elders and adults with disabilities in their community continue living independently.
Education and training of Tribal Council and informal leaders on nation building and roles and responsibilities of governments. Total grant amount is $40,640. Of that, $6,000 will be distributed using the Reimbursement Grant Process for a different purpose than the grant itself - see Funding Agreement. The remaining $34,360 is the amount of the revised grant itself.
To complete a health and wellness addition to the Porcupine District community center
